You must leave a card on file which will be charged for no-shows. This can be added in the patient portal which you will be able to access after you schedule your first appointment. (Click “Billing” link at the top of the page, then click “Payment Methods”, enter card information and click “save card”)
For new patients: Non-refundable deposit of $80 is required at the time you book your first appointment. Payment for the remaining balance for the initial evaluation is required 1 week prior to the first appointment.
For established patients: Payment is required at the time of the appointment.

Cancellations or rescheduling must be done more than 24 hours in advance.
Failure to notify of changes more than 24 hours in advance will result in a charge equal to the full fee for the appointment missed.
You may re-schedule once. Re-scheduling more than once will result in a charge equal to the full fee for the appointment.
If you frequently cancel or reschedule, I may require payment at the time of scheduling.

Check-in is required prior to each appointment.
For the first visit, check-in can be done anytime before the appointment once you establish access to OnPatient.
For follow-up visits, check-in should be done close to the appointment.
I need a photo for your profile. If you don’t provide one or it’s not clear, we’ll do one during your visit.
Photo ID is required before prescriptions can be written.

EKG may be required before prescribing stimulants.
Objective testing may be required before treating ADHD.
Urine Drug Screen (UDS) required before prescribing controlled substances.

In situations where the appointment takes longer than scheduled (ie more than 30 minutes, 45 minutes, 90 minutes) the cost of the visit will increase by $50 for every 15 minute increment.
For example, if you schedule a 30 minute appointment (usually $150) but we take 31 minutes, the charge would be $200. For the same appointment, the charge would increase to $250 at 46 minutes. This is based on the time the appointment was scheduled to start, not the actual length of the appointment.
